Travel Immunisations/Vaccinations

If you plan to travel abroad and need immunisations you will need to complete the Travel Risk Assessment form ( which can be completed below ).

We ask for the  Travel Risk Assessment Form be completed and returned to the surgery 4 – 6 weeks before your trip or longer if you are planning an extensive trip. Once we receive the completed form in the surgery a nurse will contact you to arrange an appointment, if one is required.

A charge will be made for certain immunisations and vaccinations which are not covered by the NHS.
